STEM OPT Extension
If you are interested in Optional Practical Training for international students studying in Science, Technology, Engineering or Math (STEM) areas, please complete the following:
Please gather and deliver the following documents to International Student Services in ADUC 239.
- STEM OPT Extension Request Form can be found at the ISSO in ADUC 234
- Form 1-983
- Two (2) US Passport-style Photos. They must be taken within the last 30 days. Lightly print your name and 1-94 number on the back of each photo with a pencil. Please send us scanned copies of your photos.
- Application Fee of $410. You can pay the application fee by check, money order or by credit card. Use the G-1450 form for a credit card or via check/money order.
- Complete the G-1145 form.
- Complete the I-765 form. Please enter the following Eligibility Category: (c) (3) (C) on item #27 and
- On items #28.a-28.c, list degree exactly as shown on your 1-20 and enter Employer name and "E-Verify'' Company Identification (Ask your Human Resources office for this number).
- I-94 Arrival/Departure Record
- Copies of all previously issued I-20s.
- Copy of official STEM degree transcripts or diploma.
- Copy of passport biographical page (the page that includes your name, photo, passport number and expiration date. The passport must be valid for at least six months before the application date.
- Copy of F-1 visa (citizens of Canada and Bermuda are exempt).
- Copy of any previous Employment Authorization Document (s) (EAD) (front and back)
- Copy of employment offer letter documented on the employer's letterhead.
- New OPT I-20 signed by ISS staff
